
How to Request a Transcript
The
first transcript you ever request from Capital University is furnished,
at no charge, to each student upon written request. Thereafter, it
is $5.00 for each additional copy. If you are unsure whether you have
requested before, please feel free to contact our office at (614)
236-6707. Your payment and signature must be received with request.
Transcript requests will not be processed without your payment and
signature. The request for the transcript(s) should be made to the
registrar at least one week prior to the need for the document(s).
All accounts must be paid before a transcript is issued.
If
you are requesting a transcript by mail or fax, fill out the Transcript
Request Form. Checks or money orders should be made payable to Capital
University. You may request an official transcript by fax using
a Visa or MasterCard only. We are not prepared at this time to accept
requests for transcripts via e-mail or through the web.

You may
fax your request to 614-236-6753. Please
allow 5 to 10 business days for processing once request is received.
We do offer same day service for $10.00 but must be requested in
person only. Same day in-person service is always $10.00 per transcript
regardless whether it is your very first request.
Only
official transcripts will be mailed. All transcripts mailed to the
student will be stamped "Issued to the Student" unless
otherwise requested. Photo ID is required when picking up in person.
Written permission of the student is required to pick-up a transcript
other than your own.
If
you are not using the Transcript Request Form, please submit your
request in writing including your name (any former names), Social
Security Number, date of birth, and dates of attendance. This information
will assist us in locating your transcript.
In
keeping with the Family Educational Rights and Privacy Act (1974)
official transcripts may be sent to the student or to anyone designated
by the student and are not released without written permission of
the student.
Unofficial
copies of the academic record are issued only to students currently
enrolled or at the request of the student's advisor. Please allow
a minimum of one working day for preparations of the unofficial
record.
Students
on scholarships may occasionally need an official copy of their
transcripts. The fee will be waived upon presentation of a statement
from the agency awarding the scholarship that a transcript is needed.
